    Telemedicine – use your ihealthfrontier.net logon. Its built in. ACO has options for you if you do not have a solution.

    • Most of your regular E&M codes can simply use the 95 Modifier
    • CPT 99452: ($36.88) •Interprofessional telephone/Internet/electronic health record referral service(s) provided by a treating/requesting physician or qualified health care professional, 30 minutes
    • CPT 99453: ($18.85) •Interprofessional telephone/Internet/electronic health record referral service(s) provided by a treating/requesting physician or qualified health care professional, additional 30 minutes

    Telehealth – Any electronic media works, even phone and eVigils. Here are the codes.

    • G2010 ($12.39) •Remote evaluation of recorded video and/or images submitted by an established patient (e.g., store and forward), including interpretation with follow-up with the patient within 24 business hours, not originating from a related E/M service provided within the previous 7 days nor leading to an E/M service or procedure within the next 24 hours or soonest available appointment
    • G2012 ($14.51) •Brief communication technology-based service, e.g. virtual check-in. (Scheduled, discussed triage to see if an appt is necessary)
    • 99453 ($18.85) – device setup and patient education
    • 99454 ($62.24) – supply of the device with daily recording or programmed alert transmissions
    • 99457 ($50.48) to report remote physiologic monitoring treatment management services
    • 99491 ($82.61) Create care plan for CCM
    • CPT 99446: ($18.11) •Interprofessional telephone/Internet assessment and management service provided by a consultative physician including a verbal and written report to the patient’s treating/requesting physician or other qualified health care professional; 5-10 minutes of medical consultative discussion and review
    • CPT 99447: ($35.88) 11-20 minutes
    • CPT 99448: ($53.99) 21-30 minutes
    • CPT 99449: ($71.76) 31 minutes or more
